2018 Annual Pastor's Report Instructions
Deadline: May 7, 2018

Greetings Pastors and Office Secretaries,

As our church year draws to a close, we look to report what has been happening in our churches in the past year. We report in accordance with our polity and for accountability to our district assembly. Please read this letter completely as it provides you the tools to complete your Annual Report in its entirety on time. There is a change in the qualitative report this year, so please advise. I appreciate each of you as you complete this essential piece of ministry! If you have any questions, please let me know.

YEAR-END REPORTING : Deadline for All Reports Is Monday, May 7th, 2018

1. The Annual Pastor’s Report (APR)

a.  If you have not yet set up a Naz ID, please send me your email address so this can be done right away to give you access to your church’s APR. If you are in a new assignment and the church where you are serving is not listed, please let me know. Your password should be the same as it has been in the past. If you forget your password, you will need to click the link to reset your password. (Only one address is necessary, but you may include a second one for yourself OR for another person you name to access the APR. There is a maximum of two addresses total.)

b. The email address(es) you provide will each receive a separate invitation to register for the APR. Please respond to the invitation promptly because the invitation has a limited shelf life and will expire.

c.  You can find the Annual Pastor’s Report at https://apr.nazarene.org/apr/. Please complete your report online by the deadline. Your information will be available to the district through that medium. Your compliance is essential for a completed District Assembly handbook and journal.

d. Once you have completed your report, please print your report for your own records. Do not send a physical copy to me or the District Office.

e.  Handbook Qualitative Report

·   Each pastor needs to write out a 1 page typed double spaced report (with a font of Times New Roman 12). These reports will be published in the District Assembly handbook!
·   These reports must be submitted to me electronically at mmslater08@gmail.com. The deadline for these reports is the same as with other reports: Monday, May 7th, 2018.
·   **NEW FOR 2018**: Please consider a verse from Scripture that is shaping your life or your ministry.  Your verbal report to the assembly will be to read or recite this verse (or couple of verses) to the assembly.

2. All Other Reports attached (must be submitted via e-mail or postmarked by May 4)

• Church Directory Form. Please only fill in where there are changes to last year’s form. I have attached a copy of last year’s church directory file for you to check your church. Please check addresses, email addresses, and phone numbers carefully. If there are no changes to last year’s form, please send in a blank form so that I know that you have completed it.

• Church Certificate of Election Form. Please fill in the delegates names and email addresses for assembly. Use the chart on the left to determine your number of delegates. Please note, do not just assume that it will be the same number as last year, especially if your membership total has changed.

• Deceased Church Members Form. Please fill out, sign and mail the form even if you have no deceased members to report. This way I will know that you have just not sent it in yet.

• General Superintendents’ Growth Award Form. (Contact Matthew Slater immediately if you will be applying for this award as it requires more time to process. Qualifications are listed on the form to see if your church is eligible.)

Complete these forms on your computer, sign them, and return via e-mail (PREFERRED METHOD, you can sign electronically) or mail by May 4, 2018.

Annual Reports for Ministers Not Serving as Pastor

Dear District Licensed Ministers, Associate Pastors, Retired Pastors, Lay Ministers, and other Ordained Ministers,

The time has come for you to submit your Annual Report. While the church of your membership submits its own report, you are still required to complete an Annual Report, detailing what you have done in ministry in the past year. These reports keep all of us accountable to our church as we share what God is doing in our ministries. If you do not submit a report, please understand you are subject to action by the Credentials Board that would cause a change in your credential status (Manual Paragraphs: 532.8, 536.2, 538.9).

Please complete the attached/linked form (specific to your ministry) and submit to me by e-mail by Monday, May 7, 2018. I have provided Microsoft Word and .pdf formats for convenience. You only need to complete and return one of these by e-mail. THIS METHOD IS THE PREFERRED METHOD. If you are unable to e-mail, use the .pdf of your report, print and complete, and send via mail to 3924 High St. NW Warren, OH 44483. It must be postmarked by Friday, May 4, 2017. If you are applying for the renewal of your district license, you should have completed this already, please make sure you have.
